Click HERE to start your account and apply.

Click REGISTER in the top right corner of the screen. Enter your information to begin the registration account. Be sure to select "Patient" in the drop-down menu. That will give you the option to enter your social security number and date of birth.

Once this has been done check for an email sent to you from MMMP. This is to verify your account. Once verified, you can begin to fill our your application. (Check your spam email)

Approval

You will receive your medical cannabis card via your email.  You will have the ability to download and print your card.

Follow the link in your approval email to download your license. You can also log into the 
MMMP portal by clicking HERE.

Please have your Missouri ID and card-present when visiting any of the licensed Missouri dispensaries.

We would highly recommend you have your license easily available for law enforcement/security when you are traveling. FedEx, Office Depot, local library, and other sources can print these out if you don't have access to a printer.

Those who applied for cultivation will receive a separate email from MO DHSS containing their cultivation authorization. Don't forget to display this in your grow area.

 

  • Can physicians certify medical marijuana patients via telemedicine?
    If the standard of care does not require an in-person encounter, and if the physician can conduct an exam via telemedicine in a manner that allows that physician to truthfully answer all attestations on the physician certification form in the affirmative, then yes, the physician may certify patients through telemedicine. As always, physicians licensed in Missouri who practice telemedicine must comply with RSMo 191.1145 and 191.1146. Physicians should also note the certification form requires a handwritten signature.
  • Which medical professionals can certify patients to obtain an identification card?
    Per Article XIV of the Missouri Constitution, only physicians who are licensed and in good standing to practice medicine or osteopathy under Missouri law may certify qualifying medical conditions in patients. The practice of medicine or osteopathy means practice by persons who hold a physician and surgeon license pursuant to Chapter 334, RSMo, including those who are admitted to practice in Missouri by reciprocity pursuant to section 334.043, RSMo.
  • Why are only MD and DO listed on the physician certification form?
    Article XIV of the Missouri Constitution and our rules only allow an MD or a DO to certify qualifying medical conditions.
  • What documents will I need?
    Valid Missouri ID Certification Form (paper or digital) Valid and Accesible Email Address (Very important as your account will be registered via email) Photo (or selfie) in front of white wall.
  • What fees are associated with applying for a medical cannibas card?
    Standard Patient: $25 (plus tax) This is for your standard patient card. This will give you the ability to purchase up to 4 ounces of marijuana per month. Cultivation Card: $100 (plus tax) This is if you intended to grow for either yourself or another patient. You will have the ability to grow up to 6 flowering plants (per patient) and still purchase up to 4 ounces of marijuana a month.
  • What conditions qualify?
    Cancer; Epilepsy; Glaucoma; Intractable migraines unresponsive to other treatment; A chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with multiple sclerosis, seizures, Parkinson’s disease, and Tourette’s syndrome; Debilitating psychiatric disorders, including, but not limited to, post-traumatic stress disorder, if diagnosed by a state-licensed psychiatrist; Human immunodeficiency virus or acquired immune deficiency syndrome; A chronic medical condition that is normally treated with a prescription medication that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication; A terminal illness; or In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to, hepatitis C, amyotrophic lateral sclerosis, inflammatory bowel disease, Crohn’s disease, Huntington’s disease, autism, neuropathies, sickle cell anemia, agitation of Alzheimer’s disease, cachexia, and wasting syndrome.

  • If I have a medical card can I grown my own?
    The answer is yes. When applying for your medical cannabis card you can also apply for a cultivation card with an additional fee of $100 to DHSS. This will allow you to grow up to six flowering plants in an enclosed secured space.
